With continued pressures facing the NHS, Urgent Community Response Services (UCRS) have never been needed more. Since their introduction in the NHS Long-Term Plan, Urgent Community Response Services have played a pivotal role in reducing hospital admissions & delivering urgent care within a patient’s own home.
In this conference we will explore how a diverse range of healthcare professionals have developed effective solutions within Urgent Community Response teams to tackle day one emergency care and reduce A&E admissions and hospital stays.
“Our UCR teams are having a real impact on patients, supporting them to stay in their own homes and avoid unnecessary or prolonged admissions to hospital.”
Hollie Poole, Divisional Director of Unplanned Care at Sussex Community NHS Foundation Trust, December 2024
“Receiving care at home means patients are more comfortable, and don’t have to be admitted to already busy hospitals. And there are so many conditions which can be managed by UCR, because they are made up of multidisciplinary teams, including doctors, nurses and physiotherapists.”
Dr Paula Cowan, Regional Medical Director of Primary Care, NHS England – North West
Tackling A&E wait times has been outlined as part of the government’s ‘Plan For Change’, with the NHS already putting in place a number of measures including:
“Dispatching urgent community response teams - which can treat people in their homes to relieve pressure on hospitals. Through these teams, older people and adults with complex health needs who urgently need care can get fast access to a range of health and social care professionals within 2 hours. This includes access to physiotherapy and occupational therapy, medication prescribing and reviews, and help with staying well-fed and hydrated”
